If you come to External Consultation

  • In order to schedule first appointments it is necessary to bring the doctor’s appointment application form. You can come personally to be scheduled or you can make an appointment calling 973 727227 from 8.00 am to 5.00 pm.
  • For further appointments do not forget to go to the Patients Management Area when you leave consultation or make an appointment calling 973 727227.
  • Always remember to bring your schedule card and your personal medical care card.
  • It is also necessary that patients derived from the Franja d’Aragó bring the doctor’s appointment application form and the personal medical care card from the Medical Area corresponding to the patient’s normal place of residence.
  • For appointments and medical tests derived from private insurance companies it is necessary to bring an authorisation from the responsible area in the insurance company.

If you come to have complementary examinations taken

  • Bring the examination application form.
  • Follow the instructions given for tests, X-rays or any other type of prescribed examination. 

 

What to do if…

You have to be hospitalized

You will have to formalize your admission at the Admissions Unit in the Patients Management Area. The documents you have to bring are:

  • Admission application form.
  • National Identity Card or Passport.
  • Personal Medical Care Card (CatSalut) or any other medical care authorisation .

When the admission takes place, you will be given the necessary documents and information.

 


Hospitalisation

Admissions

In order to make your admission effective you need to carry out the administrative procedures.

Where to go

Contact the Admissions Department located in the ground floor.

  • They will process your admission .
  • You will be assigned a bed.
  • They will indicate you the unit and medical speciality you belong to.

What you need to bring

  • National Identity Card or Passport.
  • The doctor’s form indicating the hospitalisation.
  • Personal Medical Care Card (CatSalut) or any other medical care authorisation.

Customer Service Hours

  • From Monday to Friday from 8.00 am to 10 pm.
  • Saturdays and Sundays from 10 am to 12 am and from 6 pm to 8 pm.

 


 

Once your admission at the Admissions Department has been formalized, you will be shown the location of your assigned room.

The rooms are shared rooms. We beg you to respect the rules of conduct and do not use your room partner’s belongings or furniture.

The medical information will be given by the doctor at the established times in each of the hospitalization units.

Silence is very important in our hospitals. Resting during the day and night positively influences the patients recovery.

We beg you use a low pitch and be also careful and considerate with your mobile phones and the TV sets volume.

You need to bring sleepers and personal hygiene items.

The hospital will remain closed at night so as to guarantee the centre’s security. Should you need to access, ring the doorbell at the main entrance door.

Bells

Every room contains two types of bells:

The first one is located next to the bed. If you ring the bell, the nurses staff will get in contact with you through an intercommunication system which is next to the door and which is for the exclusive use of the health personnel.

The second bell is located inside the bathroom and it is a red string which must be only used in case of emergency.

Smoking

According to Law 28/2005 December it is not permitted to smoke in any hospital area.

Discharge

It is only authorised by the doctor.

When you are discharged you will be given a medical report which includes a summary of the disease and the treatment you will have to go through.

You must leave the room before 12 am.

Before leaving the Hospital we will give you information regarding your medical discharge and, should you needed it, the external consultation appointment.

Voluntary discharge

If you decide to leave the hospital without the doctor’s consent you will have to fill in a voluntary discharge form assuming responsibility.

Meet and identify our staff

Both the uniform and the identity badge allow citizens to identify which work group the Medical and Management staff belong to.

Our name, post as well as a colour is printed on our badges. That way you will be able to identify who is attending you every time.

 


 

Information

You can ask for information at the main entrance desk.

Coffee shop – restaurant / Canteen

The companions and vistors can use the services offered at the Coffee shop-restaurant.

Lunch: From 1 pm to 3.30 pm. Dinner: From 8 pm to 9.30 pm.

The coffee shop will be open from 8 am to 9 pm.

There are plenty of vending machines in every floor at your disposal.

Means of transport

The public bus service has a stop in front of the Hospital (Line 9), at Alcalde Rovira Roure Avenue and at Montserrat Roig street.

Telephones

There are telephone boots at your disopsal in every floor from which you can make external calls. Every room has a telephone and a telephone number. Should you need to call, you have to dial number 9 which will provide you telephone line.

Television

There is a payment TV in every room. In order to avoid bothering the room partner we beg you not to bring any other electronic devices.

Cash dispenser

It is located at the main entrance.

Press

The local press is distributed in all rooms daily. There are free newspapers available at the Hospital main entrance.

Visits

There is an established times for visits which is from 3.30 pm to 7 pm. To preserve everybody’s well-being we recommend you do your best to try not to coincide with more than 2 people per patient.

Collaboration

The customer’s collaboration is indispensable for his or her healing. We beg you preserve and take care of the facilities and the hospital material.

Food

Food, sweets, drinks and any other type of drinks are not allowed for patients in Hospital. The schedules for meals are the following:

Breakfast From 8,30 am to 9,30 am
Lunch From 12 am to 13 pm
Tea From 4 pm to 5 pm
Dinner From 7 pm to 8 pm
After dinner From 11 pm to 12 pm

 

Safe

There is a safe at your disposal at the hospital. The Hospital does not take any responsibility for anything inside the safe.

Personal items

We recommend you not to bring jewels or money unless it is absolutely necessary, for the Hospital is not responsible for its loss. If you use dentures we beg you take care of it, especially if you have to go under surgery.

Hairdresser’s and barber’s service.

In order to ask for this service you must ask the infirmary staff first which is located in the first floor.

RIGHTS AND DUTIES

RIGHTS

  • Citizens have the right to enjoy health care and the corresponding technical and human resources or services and can not be discriminated for any reason of birth, race, gender, ideology, religion or any other personal or social condition or circumstances.
  • Children, the elderly, the mentally ill, people with chronic and crippling diseases, and those belonging to specific groups recognised as a health risk, have rights, taking the available media and resources into account, to specific actions.
  • The patient have the right to be attended to as promptly as possible, and according to equity-based criteria, suitability and availability of resources, type of pathological condition, emergency priority and a reasonable pre-established waiting time.
  • It must be respected human personality, dignity and privacy, with no type of discrimination.
  • The patient has the right to continue to maintain his relationship with relatives.
  • The person has the right to know suitable and understandable information on their health-care process.
  • It must be respected patient’s privacy and confidentiality of information about health process and clinical history.
  • A written consent must be given to participate in research and experimentation studies.
  • Health evaluation must be social addressed.
  • The patient has the right to request a second opinion or choose another health professional.
  • The patient can reject staying in a hospital or health establishment whenever they need.
  • Every individual has the right to live through the process they are affected by until death, with dignity.
  • In the exercise of his autonomy, the patient must be informed to grant his consent for diagnostic test, preventive action or treatment. Informed consent means the acceptance of a procedure by a patient after the latter has been furnished suitable information and in time enough to be able to participate freely in the decision.
  • The user has the right to know and use procedures of submitting suggestions and complaints. They must be evaluated and answered.
  • The patient has the right to access the documentation on his clinical history.
  • In the advance directive document, an adult with sufficient capacity, and of his own free will, issues the instructions to be taken account when he is in a situation in which the prevailing circumstances render it impossible for him to personally express his own will, in accordance to Law 21/2000 of December 24.
  • Patients have the right to know and identify the professionals providing the health care.

DUTIES:

  • Citizens must make use of the resources services and rights according to their health needs and depending on availability in the health system.
  • The patient is obliged to accept discharge once the health-care process the centre or the unit can over is complete.
  • Patients have the duty to respect the rules and regulations established in each hospital.
  • Citizens must respect material resources and health facilities of the hospital.
  • Patients have the duty to respect the personal and professional dignity of the personnel employed and the other patients.
  • The patient has to sign the relevant document if they refuse to any health action proposed.
  • Citizens must look after their health and be responsible for/with it.
  • The citizen must provide, truthfully, his identification data and any data pertaining to his physical condition or health.
  • Patients must respect and comply with health measures adopted for the prevention of risk.
